How does Clopidogrel / Generic Chemic work?
Clopidogrel causes sticking of platelets to the damaged vessel wall and it aggregates to form a platelet plug. The platelet plug releases ADP and thrombaxane A2 (TXA2). In veins due to the sluggish blood flow a fibrinous tail is formed that traps RBCs and in arteries the thrombus is due to platelet plug. Chemic alters the receptors on platelets and inhibits ADP and fibrinogen induced platelet aggregation. The purinergic receptors which mediate adenylyl cyclase inhibition by ADP are blocked by Clopidogrel. This prevents activation of platelets. Chemicl also prevents fibrinogen binding to platelets but does not affect TXA2.

The common side effects reported with Clopidogrel are chest pain, influenza like symptoms, pain, fatigue, edema, hypertension, head ache, dizziness, abdominal pain, dyspepsia, diarrhea, nausea, hypercholesterolemia, back pain , purpura, depression, upper respiratory tract infection, bronchitis, coughing and urinary tract infection.

Guidelines before using Clopidogrel
The concomitant use of non-steroidal anti-inflammatory drugs (NSAIDs) and Chemic should be avoided as it can cause occult gastrointestinal blood loss. The co-administration of Chemic and warfarin should be done with caution, as there is a risk of increased bleeding. Chemic should be used with caution in patients with renal or hepatic impairment. Clopidogrel should not be administered to pregnant women. If it is an absolute must to take Chemic during pregnancy, it should be administered under the supervision of a medical practitioner. Clopidogrel should not be administered to nursing mothers and in pediatric patients. Chemic is contraindicated in patients with peptic ulcer or intracranial hemorrhage.
