How does Linezolid / Generic Zyvox work?
Linezolid inhibits bacterial protein synthesis by binding to 23S fraction of 50S ribosome and interferes with formation of the ternary N-formylmethionine-tRNA-70S complex. The binding of Linezolid distorts the tRNA site overlapping both 50S and 30S ribosomal subunits and stops protein synthesis before it starts.

The common side effects reported with Linezolid are diarrhea, nausea, head ache, vomiting, insomnia, constipation, rash, dizziness, fever, taste alteration, abnormal liver function tests, tongue discolouration, vaginal moniliasis and oral moniliasis.

Guidelines before using Linezolid
The concomitant use of Linezolid with adrenergic and serotonergic agents is not recommended.  Zyvox can cause lactic acidosis and convulsions. Zyvox when administered along with selective serotonin reuptake inhibitors (SSRIs) can cause serious serotonin syndrome. The long term use of Linezolid can cause peripheral and optic neuropathy.  The prolonged use of Zyvox can result in the development of superinfection. It is not advisable to consume the Linezolid during pregnancy and lactation. Linezolid should not be administered to patients with uncontrolled hypertension, pheochromocytoma and thyrotoxicosis. The use of directly and indirectly acting sympathomimetic agents (e.g. pseudoephedrine), vasopressive agents (e.g. epinephrine, norepinephrine), dopaminergic agents (e.g. dopamine, dobutamine) is contraindicated along with Linezolid. Linezolid can be taken without regard of food. The patient should be advised to avoid foods or beverages with high tyramine content while taking Zyvox.  Zyvox should not be used in patients who are on monoamine oxidase inhibitors or within 2 weeks of discontinuing MAO inhibitors.
